A 45-year-old woman, no known underlying disease
CC: ไอ 1 เดือนก่อนมา รพ.
Key clinical findings:
1 เดือนก่อน ไอแห้ง เหนื่อยเวลาไอมากและออกแรง นอนราบได้ ไม่บวม ไม่มีไอเป็นเลือด น้ำหนักลด 3 กก.ใน 1 เดือน
10 วันก่อน ไอมากขึ้น เสมหะน้อย ไม่มีเลือดปน เหนื่อยมากขึ้น
Key investigation:
Diffuse patchy ground-glass and consolidation with intralobular septal thickening (crazy paving pattern) in both lungs, predominates in dependent part of right lung
Multiple vary in size, ill-defined centrilobular ground-glass nodules in both lungs
Lobar consolidation of both lower lungs containing ill-defined enhancing mass size 3.7×3.6×3.8 cm.
Differentail diagnosis
- Lung cancer (bronchoalveolar carcinoma)
- Pulmonary lymphoma
- Pulmonary tuberculosis แต่ไม่อธิบาย mass lesion ที่ LLL
- Organizing pneumonia แต่ไม่อธิบาย mass lesion ที่ LLL
- Pulmonary alveolar proteinosis (PAP) แต่ไม่อธิบาย mass lesion ที่ LLL
- ไม่คิดถึง Bacterial pneumonia เนื่องจาก onset นานเกินไป
Bronchoscopy
- BAL fluid culture: no growth all
- Transbronchial lung biopsy: Mucinous adenocarcinoma, lepidic growth pattern
